Please describe the content of the item(s) being sent in as much detail as possible in a language understood in the addressee country.

For shipments for personal use, be sure to indicate “Personal Use.

Enter the quantity, weight, and price of the contents. (Please use the “¥” symbol in front of the price.).

When shipping a gift or a product sample, enter “X” in the corresponding box.

Select a shipping method by entering "X" in the corresponding box.

If you wish to insure your mail, put a checkmark in the box and enter the insurance value.

Select the method for handling your mail in the event it cannot be delivered to the recipient by entering "X" in the corresponding box
